Physical Activity Release of Liability Form
The Next Level Soccer CT organization requires that players with existing health risks work with their physicians to ensure that their participation with Next Level is compatible with their existing treatment plan and does not pose an unacceptable risk to their ongoing health condition and/or recovery.
I hereby acknowledge that my child's participation in physical activities pertaining to soccer training may involve but are not limited to the following:
Running, jumping, kicking a soccer ball, controlling a kicked or thrown soccer ball, limited heading of a soccer ball, player to player contact (tackling), stretching for, with and without a soccer ball etc.
By signing this form I recognize and acknowledge my understanding that my child's participation in the aforementioned activities may expose my child to certain foreseeable and unforeseeable risks of damage and/or bodily injury, including serious bodily injury, where he/she may need to be hospitalized.
I knowingly, freely and voluntarily assume all risks associated in participation of the above mentioned activities.
I hereby release Next Level Soccer CT and its subcontractors from any and all liability arising out of my child's participation of the aforementioned activities and hereby waive my rights herein to assert any claim(s) for damages, bodily injury or serious bodily injury to the fullest extent allowed by law.
I understand, acknowledge and accept that this release is intended to be as broad and inclusive as permitted by Connecticut state law.

I grant Next Level Soccer CT to use photographs and video taken for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.
